Full payment is required at the time services are provided and prior to discharge. We accept cash, check, VISA, MC, AMEX, Discover, Debit and Care Credit. I understand that the hospital staff will provide an estimate of current and anticipated charges at time I request one, By signing below, I’m requesting that veterinary care be provided for pets presented by me or my agents. I understand that I am financially responsible for all services provided. There will be a 1.5% monthly service fee on all unpaid balances. From time to time we love to take photos of the pets we take care of. Please give us permission to use these photos on our social media sites by signing below. *

Cancellation Policy: We would like to thank you all for your loyalty and dedication to our practice. As our practice grows, keeping a timely schedule is our shared priority to provide the highest quality of care to every critter that makes its way through our doors. Therefore, we are implementing a 24-hour cancellation policy. If you need to cancel or reschedule your appointment, please notify us at least 24 hours before the appointment. If you do not contact us and you do not show up for your appointment, you will be charged a no-show fee. No-show fees are as follows: Appointment: $50.00 Surgery: $100.00 *

New Client Exam Charge: For new client(s) seen for the first time at Malvern Veterinary Hospital an exam fee is expected at the time of scheduling to hold an appointment. Fees are non-refundable if not canceling 24 hours ahead of time, though appointments can be rescheduled. New Client Exam Charge is $94.00. We apologize for any inconvenience this may cause and appreciate your understanding. *
